Take a close look at this application before you use it. It asks for the applicant's cell phone number rather than the work phone number. It asks for an email address. It does not ask for date of birth. It does not ask for the relationship or age of other persons who will be living with the applicant. It does not ask for the applicant's occupation. It does ask for the last four digits of banking and credit account numbers.
NOTICE TO PAY RENT
Here's a notice to pay rent or quit which incorporates certain payment information (when, where, and to whom the rent must be paid).
FORM FOR TENANCY REFERENCE
This form is actually a letter which begins "Fellow Landlord:" and goes on to ask seventeen questions about a tenant who's being screened. The letter could be mailed along with a self-addressed, stamped envelope, or it could be used as a guide for, and record of, a telephone inquiry.
